UAMS Covid-19 Email Address or Google Form

Students are not returning for classes to the school building until January 18th. All students must be tested for Covid-19, and the results must be submitted to an email address OR a Google Form. (It is not necessary to complete the form AND send an email. Please ONLY use ONE option to provide the result). If you choose to email the Covid Result, please ensure the student name, grade and an image of the test result is included in the body of the email.
